MOUNJARO is a prescription-only weight management treatment for adults aged 18 and over with a BMI of 30 or above, or a BMI of 27 or above with at least one weight-related health condition.

NEW PATIENTS — you must complete your assessment first
This is a prescription-only medicine. It will not be dispensed or dispatched until our prescriber has assessed your suitability.
Complete your Weight Management Assessment
Once submitted, return to this page to select your dose and check out. We will review your assessment as soon as we can. If you are not suitable for treatment we will contact you and refund your payment in full — nothing is dispatched before your prescriber has approved it.
 
EXISTING PATIENTS
If you have already been prescribed Mounjaro by us, you do not need to complete the assessment again. Select your dose and check out as normal.
But you must tell us if anything has changed. That means any new health condition or diagnosis, any change to your medications, any side effects that have concerned you, or if you are pregnant, breastfeeding or planning a pregnancy.
 
Please email info@sebastianrose.co.uk or call 0800 772 0137 before placing your order. In most cases it will not change anything — but the decision needs to be ours, not assumed.
If we hear nothing from you, we will proceed on the basis that nothing has changed. Keeping us informed is your responsibility, and we cannot be responsible for harm arising from information we were not given.
