Cellulite is one of the most researched, most marketed and most poorly treated conditions in aesthetics. Billions are spent globally every year on products and procedures claiming to reduce it — the vast majority of which have no meaningful clinical evidence behind them. Creams, massage, radiofrequency devices, cryotherapy, fat dissolving injections — all have been marketed for cellulite. Most produce marginal results at best.
In December 2025, something genuinely significant happened.

Galderma — the Swiss dermatology company behind Sculptra — received European Union Medical Device Regulation (MDR) certification specifically expanding Sculptra's formal clinical indication beyond the face to include the gluteal area and posterior thighs. Critically, cellulite improvement was explicitly included in the supporting clinical data submitted for this certification.
This matters more than most people realise. Here is why — and what it means for patients in Cardiff and across Wales who have been looking for a treatment that actually works.
What Is EU MDR Certification and Why Does It Matter?
The EU Medical Device Regulation is the most rigorous medical device regulatory framework in Europe. It replaced the previous CE mark system and introduced substantially stricter requirements for clinical evidence, post-market surveillance and product safety monitoring. Obtaining MDR certification for a new clinical indication is not a simple administrative process — it requires the submission of robust clinical data demonstrating safety and efficacy in the specific application being certified.
When Galderma submitted Sculptra for MDR certification for body indications — including cellulite improvement on the posterior thighs and gluteal area — they did so on the basis of multiple peer-reviewed clinical studies, randomised controlled trial data and long-term patient outcome evidence. The certification was granted on 5 December 2025.
This means that Sculptra for cellulite is not an off-label treatment, not an experimental application and not a repurposed facial product. It is a formally certified, evidence-backed clinical treatment for cellulite — the first injectable biostimulator to hold this status in Europe.
Why Most Cellulite Treatments Don't Work

To understand why Sculptra's MDR certification is significant, it helps to understand what cellulite actually is — because most treatments fail because they address the wrong problem entirely.
Cellulite is not caused by excess fat. It is caused by a structural failure in the tissue beneath the skin — specifically, the progressive deterioration of the extracellular matrix, the thickening of fibrous septae (the connective tissue bands that attach skin to underlying muscle), disruption of microcirculation, tissue acidosis and the progressive loss of collagen and elastin in the dermis overlying the affected area.
These structural changes produce the characteristic dimpled, uneven skin surface that most people recognise as cellulite — regardless of body weight or fitness level. Studies consistently show that over 85% of post-pubertal women are affected, across all body types.
This structural origin is why topical creams, massage, fat dissolving injections and most device-based treatments produce limited results. They either do not penetrate deeply enough, do not address the underlying structural deficit, or both. Fat dissolving treatments — however effective for fat reduction — do not address fibrous septae or collagen loss. They treat a different problem.
Effective cellulite treatment requires addressing the collagen framework and structural integrity of the tissue itself. This is precisely where Sculptra operates.
What Is Sculptra and How Does It Work?

Sculptra has been used in aesthetic medicine since 1999 — originally developed for facial volume restoration and wrinkle correction. Its active ingredient is poly-L-lactic acid (PLLA), a synthetic biodegradable polymer that has been used safely in medical implants, surgical sutures and dissolvable stitches for over 30 years.
When Sculptra is injected into the subcutaneous tissue, the PLLA microparticles stimulate a controlled biological response — activating the local fibroblasts (the cells responsible for producing structural proteins) and triggering the production of new Type I and Type III collagen. As the PLLA microparticles are gradually metabolised and eliminated by the body over several months, new collagen fills the space they occupied — rebuilding the structural scaffolding of the treated tissue from within.
This is fundamentally different from hyaluronic acid fillers, which simply add volume by filling space with a hydrating gel. Sculptra does not add volume directly — it stimulates your body to rebuild its own collagen. The result is tissue that is structurally stronger, firmer and more resilient, with improved skin quality that looks and feels entirely natural because it is entirely natural. It is your own collagen.
The clinical significance for cellulite is substantial. By rebuilding the collagen framework beneath the affected skin, Sculptra progressively improves the structural integrity that cellulite destroys — thickening the dermis, improving skin firmness and elasticity, and reducing the prominence of the dimpling that results from the underlying tissue deterioration. It does not release fibrous septae mechanically, but it strengthens and remodels the tissue environment in which they exist — producing progressive, cumulative improvement in the overall appearance of the skin surface.
The Clinical Evidence — What the Studies Actually Show
The MDR certification was supported by multiple clinical studies. The published data is compelling.

In clinical studies focused on the posterior thighs — the primary cellulite treatment area — visible improvements were seen as early as one month after the first treatment session. At one year of follow-up, 100% of patients noted visual improvements to their thighs. 93% reported reduced sagginess. 97% expressed satisfaction with the appearance of their thighs. 83% indicated they would choose to undergo the treatment again.
For the gluteal area, 84% of patients were rated as having improved or better by physicians using the validated Global Assessment of Improvement Scale at six months. 96% of patients reported high satisfaction.
These are not subjective marketing claims. These are peer-reviewed clinical outcomes from the studies submitted to regulators for MDR certification — including a prospective multicentre trial published in the Journal of Drugs in Dermatology and a pilot study presented at the IMCAS World Congress in Paris evaluating PLLA specifically for cellulite improvement in the lower extremities.
No other injectable treatment currently available in the UK or Europe holds comparable formal regulatory certification for cellulite with this level of supporting clinical evidence.
The Treatment Protocol — What to Expect
Sculptra for cellulite at Sebastian Rose follows the Galderma-endorsed protocol established through the clinical trial evidence.
Sessions: Three sessions are the standard protocol, spaced approximately four weeks apart. Some patients with milder grade I to II cellulite may achieve satisfactory results in two sessions. Patients with more advanced grade III to IV cellulite may benefit from a fourth session.
Product: For body applications, Sculptra is reconstituted at a higher dilution than facial treatment — typically 16ml per vial compared to 8ml for the face. This creates a more fluid, spreadable product that distributes evenly through the larger surface area of body skin.
Vials per session: The clinical trial protocol used up to four vials per session for combined thigh and buttock treatment — two vials per side per area. Your practitioner will determine the correct number of vials for your anatomy and treatment area at consultation.
Results timeline: Improvements begin to develop within four to six weeks of the first session as the collagen stimulation response begins. Progressive improvement continues for three to six months after the final session as new collagen continues to be produced. Peak results are typically visible at six months following the course of treatment.
Duration of results: Clinical data demonstrates results lasting two years or more for body applications. Because Sculptra stimulates your own collagen production rather than introducing a temporary filler, the results are structural and genuinely long-lasting. Maintenance treatment every one to two years is recommended to sustain results as the natural ageing process continues.
Treatment areas: Posterior thighs (both sides), buttocks (both sides), and combinations of these areas. The knees can also be treated where appropriate.
Sculptra vs Other Cellulite Treatments — An Honest Comparison
Given the number of treatments marketed for cellulite, it is worth being direct about where Sculptra sits clinically relative to the alternatives.

Topical treatments: No topical product penetrates the dermis sufficiently to address the structural changes that cause cellulite. They may temporarily improve hydration and skin surface appearance. They do not produce meaningful clinical improvement in cellulite. None have MDR or equivalent certification for cellulite.
Fat dissolving injections (deoxycholic acid / Lemon Bottle): These treat localised fat deposits. Cellulite is not primarily caused by fat — it is a structural and connective tissue condition. Fat dissolving injections have no peer-reviewed clinical evidence for cellulite improvement and are not indicated for this purpose. Radiofrequency devices: Some RF devices have reasonable clinical evidence for temporary cellulite improvement through thermal remodelling of collagen. Results are typically modest and not long-lasting. We do not currently offer RF body treatments at Sebastian Rose.
Sunekos Cell 15: This is the only other treatment we offer for cellulite — because it is the only other injectable with peer-reviewed published clinical evidence for this indication. It works through a completely different mechanism to Sculptra — addressing the extracellular matrix, tissue acidosis and microcirculation rather than the collagen framework. The two treatments are complementary and a combination protocol addressing both mechanisms simultaneously is the most comprehensive approach we offer.

The Combination Protocol — Sunekos Cell 15 and Sculptra
For patients with moderate to significant cellulite, we typically recommend addressing it from two biological angles simultaneously — using both Sunekos Cell 15 and Sculptra as a combination protocol.
Sunekos Cell 15 addresses the extracellular matrix dysfunction, tissue acidosis and microcirculatory disruption that drive cellulite from within the affected tissue — working at the level of the ECM to restore the biological environment that healthy skin requires.
Sculptra works at a deeper structural level — rebuilding the collagen scaffolding beneath the skin that cellulite progressively destroys, progressively thickening the dermis and improving skin firmness and resilience over the following months.
The two treatments do not compete — they operate at different biological levels and in different timeframes. Sunekos Cell 15 works earlier in the treatment course, improving the tissue environment. Sculptra's collagen stimulation builds progressively over three to six months. Used together, they produce a more comprehensive and durable clinical result than either achieves independently.

Sculptra for cellulite is most effective for patients with grade I to III cellulite on the posterior thighs and buttocks — where the dimpling is present at rest or with gentle compression and the underlying skin laxity is a contributing factor. It is particularly well suited to patients whose cellulite is driven primarily by skin quality deterioration and collagen loss rather than very deep structural fibrous band tethering.
It is not a weight loss treatment. It does not reduce fat. Patients should be at or near their target weight before undergoing treatment for the best clinical outcomes.
