What is Sculptra?
Sculptra is not a dermal filler. It is a collagen biostimulator — one of the most advanced anti-ageing injectables available in medical aesthetics today, and one of the most misunderstood.
Where traditional hyaluronic acid fillers add volume by physically filling an area, Sculptra works differently and more profoundly. It uses poly-L-lactic acid (PLLA) — a biocompatible, biodegradable substance that has been used safely in medicine for decades in dissolvable sutures and surgical mesh — to trigger your body's own fibroblast cells to produce new collagen. HOW DOES SCULPTRA WORK?
How Does Sculptra Work?
Sculptra contains tiny microspheres of poly-L-lactic acid (PLLA) suspended in sterile water. When injected into the deep dermis, these microspheres act as a scaffold — stimulating the surrounding fibroblast cells to begin producing new collagen fibres around them. Over the following weeks and months, as the PLLA is gradually and safely absorbed by the body, the newly produced collagen remains — restoring volume, structure and firmness from within.
The process happens in three stages:
Stage 1
Immediate (days 1–3) There is initial volume from the water carrier solution used to prepare Sculptra. This is temporary and resolves within a few days — this is not the final result.
Stage 2
Collagen Stimulation (weeks 2–8) The PLLA microspheres trigger collagen production. You begin to notice gradual improvements in volume and skin quality.
Stage 3
Full Results (months 3–6) Final results become visible as collagen production peaks. Results continue to improve for up to six months post-treatment and last two to three years.

Your Treatment Plan
Most clients require a course of 2-3 Sculptra sessions, spaced approximately 4–6 weeks apart. The number of vials required per session depends on the degree of volume loss and the areas being treated — this is determined individually at consultation.
The Treatment
Sessions typically take 30–45 minutes. The area is cleaned and a topical numbing cream may be applied for comfort. Sculptra is reconstituted with sterile water and lidocaine to minimise discomfort, then injected using a fine cannula or needle into the deep dermis. Massage is performed during and after treatment — this is essential with Sculptra to ensure even distribution.
After Your Treatment
You may experience redness, swelling and mild bruising for a few days — this is completely normal. You will be given detailed aftercare instructions including the 5-5-5 massage protocol (massage the area for 5 minutes, 5 times per day, for 5 days) which is essential for optimal Sculptra results.
Follow-Up
We ask all Sculptra clients to return approximately 4 weeks after each session so we can assess collagen development and determine whether additional vials are needed.
SCULPTRA RESULTS - WHAT TO EXPECT
Unlike immediate-result fillers, Sculptra results develop gradually and authentically. Here is a realistic timeline:
Week 1–2:
Initial water volume resolves. The treatment area may look similar to before treatment — this is normal and expected.
Week 3–6:
First signs of collagen development begin. Subtle improvements in skin quality and firmness become noticeable.
Month 2–4:
Volume restoration becomes visible. Lines soften. Skin feels firmer and more supported.
Month 4–6:
Full results achieved. Significant improvement in facial volume, contour and skin quality.
Year 1–3:

How long does Sculptra last?

How many sessions of Sculptra will I need?

Does Sculptra hurt?
Sculptra is generally well-tolerated. The product is reconstituted with lidocaine (a local anaesthetic) to minimise discomfort, and topical numbing cream is available. Most clients describe the sensation as mild pressure or stinging at the injection sites.
Is there any downtime after Sculptra?
There is minimal downtime. You may experience redness, swelling and mild bruising for a few days. Most clients return to normal activities immediately. Strenuous exercise should be avoided for 24 hours.
When will I see results from Sculptra? Results develop gradually over 2–6 months as your body produces new collagen. This is by design — Sculptra is not an immediate-result treatment. The gradual improvement is what makes results look so natural.
Can Sculptra be combined with other treatments?
Yes — Sculptra combines well with Botox, hyaluronic acid fillers, Profhilo and skin booster treatments. At Sebastian-Rose we often design bespoke combination treatment plans. Your practitioner will advise what combinations are appropriate and in what order.
Is Sculptra safe?
Sculptra has an excellent safety profile and has been used in over 30 countries for decades. It is FDA-approved and uses a biocompatible, biodegradable material (PLLA) that has been used safely in medical devices for years. As with all injectable treatments, risks exist — including bruising, swelling, nodule formation and, very rarely, granulomas. These risks are minimised by correct injection technique, proper reconstitution and post-treatment massage. At Sebastian-Rose, your safety is always our first priority.
What is the 5-5-5 Sculptra massage rule?
After Sculptra treatment, you must massage the treated areas for 5 minutes, 5 times per day, for 5 days. This distributes the product evenly and is essential for preventing nodule formation. We will demonstrate the technique before you leave the clinic.
Can Sculptra cause nodules?
Nodule formation is a known but uncommon risk with Sculptra, and is significantly reduced by correct product reconstitution, injection technique and post-treatment massage. At Sebastian-Rose we follow all Galderma clinical guidelines to minimise this risk. If you notice any firm lumps after treatment, contact us immediately.
How does Sculptra compare to Profhilo?
Both Sculptra and Profhilo are biostimulators, but they work differently. Profhilo uses high-concentration hyaluronic acid to hydrate and stimulate collagen and elastin in a broad distribution pattern — it is excellent for skin quality and mild laxity. Sculptra uses PLLA to stimulate targeted collagen production for volume restoration and structural support. Many clients benefit from both. Your practitioner will recommend the most appropriate option at consultation.
Can I have Sculptra if I have had filler before?
Yes, in most cases. We will assess the location and extent of any existing filler at your consultation. In some cases we may recommend waiting for existing filler to partially dissolve before proceeding with Sculptra, to ensure optimal, natural results.
Is Sculptra available on the NHS?
No. Sculptra is a cosmetic treatment and is not available on the NHS. It is available privately at Sebastian-Rose Medical Aesthetics Cardiff.
